House to Home

House to Home supports families with medically complex children by bringing essential care, telehealth services through TytoCare, durable medical equipment, and important resources directly to their homes. A Community Health Worker aboard the mobile unit helps guide families through the process, connects them with additional support services, and helps ensure they have access to the care and resources they need.

A referral from the URMC Complex Care Center is required, and the patient must be enrolled in the complex care program.
